    ABSTRACTThis paper discusses the management of the technology transfer process conducted by a Technology Transfer Office (TTO) of a federal public university. Patent co-authorship and multidisciplinarity were used as concepts to evaluate and monitor the quality of academic and practical contribution and their potential for commercial application, using descriptive statistics and cluster analysis. Considering only multidisciplinary patents, binary cluster analysis was conducted, using Jaccard similarity measurement and single linkage method to determine proximity among academic units. Apart from the analysis of the number of patents, the approach enabled discussions and questions regarding the differences between patent generation patterns, resultant from the specific organizational culture and structures. The discussions are relevant to improve the identification of opportunities in technology transfer processes by the TTO

    Interleukin-1 as a mediator of fatigue in disease: a narrative review

    Fatigue is commonly reported in a variety of illnesses, and it has major impact on quality of life. Previously, it was thought that fatigue originates in the skeletal muscles, leading to cessation of activity. However, more recently, it has become clear that the brain is the central regulator of fatigue perception. It has been suggested that pro-inflammatory cytokines, especially interleukin-1 alpha (IL-1α) and interleukin-1 beta (IL-1β), play a prominent role in the development of central fatigue, and several studies have been performed to elucidate the connection between inflammation and these central processes.In this narrative review, mechanisms of action of IL-1 are described, with special attention to its effect on the central nervous system. In addition, we present a summary of studies that (i) investigated the relationship between circulating IL-1α and IL-1β and fatigue severity and/or (ii) evaluated the effect of inhibiting IL-1 on fatigue. We aim to improve the understanding of fatigue in both inflammatory and non-inflammatory illnesses, which could help develop strategies to treat fatigue more effectively.Reviewing the studies that have been performed, it appears that there is a limited value of measuring circulating IL-1.     A Decision Support Tool to Optimize Selection of Head and Neck Cancer Patients for Proton Therapy

    SIMPLE SUMMARY: A decision support tool was developed to select head and neck cancer patients for proton therapy. The tool uses delineation data to predict expected toxicity risk reduction with proton therapy and can be used before a treatment plan is created. The positive predictive value of the tool is >90%. This tool significantly reduces delays in commencing treatment and avoid redundant photon vs. proton treatment plan comparison. ABSTRACT: Selection of head and neck cancer (HNC) patients for proton therapy (PT) using plan comparison (VMAT vs. IMPT) for each patient is labor-intensive. Our aim was to develop a decision support tool to identify patients with high probability to qualify for PT, at a very early stage (immediately after delineation) to avoid delay in treatment initiation. A total of 151 HNC patients were included, of which 106 (70%) patients qualified for PT. Linear regression models for individual OARs were created to predict the D(mean) to the OARs for VMAT and IMPT plans. The predictors were OAR volume percentages overlapping with target volumes. Then, actual and predicted plan comparison decisions were compared. Actual and predicted OAR D(mean) (VMAT R(2) = 0.953, IMPT R(2) = 0.975) and NTCP values (VMAT R(2) = 0.986, IMPT R(2) = 0.992) were highly correlated. The sensitivity, specificity, PPV and NPV of the decision support tool were 64%, 87%, 92% and 51%, respectively. The expected toxicity reduction with IMPT can be predicted using only the delineation data. The probability of qualifying for PT is >90% when the tool indicates a positive outcome for PT. This tool will contribute significantly to a more effective selection of HNC patients for PT at a much earlier stage, reducing treatment delay

    Motivational factors mediate the association of general self-efficacy and performance outcomes in acutely hospitalised older patients

    Objectives: To study (i) the association of general self-efficacy (GSE) on the course of subjective (i.e. basic and instrumental activities of daily living (ADLs and IADLs) and objective physical performance outcomes (short physical performance battery (SPPB)) among older persons from discharge up to 3 months post-discharge and (ii) the extent to whether motivational factors such as depressive symptoms, apathy and fatigue mediate this association.Methods: Prospective multi-centre cohort of acutely hospitalised patients aged ≥70 (Hospital-ADL study). Structural equation modelling was used to analyse the structural relationships.Results: The analytic sample included 236 acutely hospitalised patients. GSE had a significant total effect on the course of subjective and objective performance outcomes (ADLs: β = -0.21, P &lt; 0.001, IADLs: β = -0.24, P &lt; 0.001 and SPPB: β = 0.17, P &lt; 0.001). However, when motivational factors as mediator were included into the same model, motivational factors (IADLs: β = 0.51, P &lt; 0.001; SPPB: β = 0.49, P &lt; 0.001) but not GSE remained significantly associated with IADLs (β = -0.06, P = 0.16) and SPPB (β = 0.002, P = 0.97). Motivational factors partially mediated the relationship between GSE and ADLs (β = -0.09, P = 0.04). The percentage of mediation was 55, 74 and 99% for ADLs, IADLs and SPPB, respectively.Conclusions: Motivational factors and GSE are both associated with subjective and objective performance outcomes. However, the relationship between GSE and subjective and objective performance outcomes was highly mediated by motivational factors. Taken together, this suggests that GSE is important to being physically active but not sufficient to becoming more physical active in acutely hospitalised older patients; motivation is important to improving both subjective and objective performance.</p
